System Access Forms

Forms for specific types of access including wireless accounts, TriNetX, and remote access are available in SelfServe (login required). Links to frequently used forms:

• Network Account Request
  Request access to departmental shares and Upstate accounts for voluntary faculty.
• Wireless Access Request
  Level of access is determined by the requestor’s role at Upstate. Please note that internal wireless is only for Upstate managed devices. Academic Wireless is provided for Students and staff with personal devices.
• Remote Access Request
  One of two options is available for remote connections to the campus network. Request Cisco AnyConnect for Upstate managed devices or Citrix VPN for personal devices.
